    Things to know before you apply:

    • You must be an Authorized Officer and controlling party of the business entity to submit this application.
    • When opening an account, Federal regulations require that we obtain, verify, and record information for Beneficial Owners of an entity customer.
    • For the purposes of this application, these are key individuals who control the company, or who own the company at a level of 25% or greater ownership.
    • If a Trust owns 25% or greater, then enter the Trustee's information as the owner.

    Employees may be added to your account as Authorized Users. You must pay us for all charges made or allowed by Authorized Users. Authorized Users are able to get account information. You also need the Authorized User’s permission to give us any information about them that we request and to allow us to share information about them.

    To add an Authorized User to your account, you'll need to know his or her:

    • Full Name
    • Date of Birth
    • Social Security Number
    • Costco Membership Number

    If you don't know this information or have it with you, you can add an Authorized User to your account later by phone or online.

    By submitting this information, you agree that the Authorized User is an employee or contractor of your business and has consented to the provision of his/her personal information in connection with this application.

    Each Authorized User must have a current Costco membership in his/her name to receive an Authorized User card. We may cancel an Authorized User's card and participation in this program if he/she does not maintain their Costco Membership.

    You may request additional cards for up to three Authorized Users right now. If approved for your card, you can request cards for more Authorized Users either by phone or online.

    • This application requires the name, address, date of birth, and social security number (for U.S. Citizens) or passport number (if not a U.S. Citizen) of the Authorized Officer and any Beneficial Owners who own 25% or more of the entity.
    • After your application is submitted, we may seek additional information about these individuals and the legal entity.

    Costco Membership Auto Renewal
    If you are not currently enrolled in auto renewal of your Costco Membership with Costco, you are the Primary Member on your Costco membership, your application is approved, and your card is activated, your Costco membership fee will automatically renew, and be charged to your Costco Anywhere Visa Card annually on the first day of your renewal month until you choose a different form of payment or cancel your Costco membership, and will qualify toward your annual credit card reward certificate. Membership fees will be charged based on your membership level ($60/yr for Gold Star and Business Members, plus a $60/yr upgrade fee for Executive Members) and $60/yr for each Affiliate Business Member, if any, plus applicable taxes. If you wish to pay your Costco membership fee with a different form of payment or cancel your Costco membership, please visit the Costco Membership Counter or call Costco at 1-800-774-2678, at least 48 hours prior to the first day of your renewal month.
